The plot of River City Ransom continues the Nekketsu line and tells the story of Kunio-kun and his classmate going to River City to rescue their friend Cindy, who has been kidnapped by the villain Slick. On their way to the bandit's lair, the friends battle various street gangs. The game is not linear and allows you to explore the world.

The gameplay of River City Ransom is consistent with the other games in the series and very similar to Double Dragon. Persianages can jump, run and strike with their hands and feet, as well as use the enemy's weapons against themselves. Special attacks the player can buy for points in the stores, which are found in each level on the way. The game supports password entry, to save the progress. The opponents are gang members, of which there are a total of five in the game. Each gang has its own color of clothing, and at the end of each level there's a battle with the boss - the leader of the gang.
